Open the Google Play Store app on your Android device. Tap the profile icon in the top right corner. Select “Payments & subscriptions” from the menu. Tap “Subscriptions” to view your active subscriptions.

Find Google Forms in the list and tap on it. Select “Cancel subscription” at the bottom of the page. Choose a reason for cancellation from the options provided. Confirm your choice by tapping “Continue” and then “Cancel subscription” again.
